This study presents a pilot program integrating AI-based risk stratification, low-dose computed tomography (LDCT), and liquid biopsy (circulating tumor DNA) to enhance early lung cancer detection among high-risk individuals, primarily smokers, in India. Lung cancer remains the leading cause of cancer deaths globally, mostly diagnosed at late stages. Early detection through screening can reduce mortality but faces challenges including low participation and diagnostic limitations.<br /><br />The approach utilizes an AI-powered questionnaire incorporating clinical, behavioral, and environmental factors to categorize 1,000 screened individuals into high (36%), intermediate (45%), and low (19%) risk groups. The questionnaire demonstrated a 53% concordance with LDCT findings, reflecting its potential as a scalable, low-cost, non-invasive frontline screening tool. LDCT identified suspicious lesions (RADS 3-4) in 15% of cases. Subsequently, 20% of high-risk participants underwent liquid biopsy, adding molecular precision to detection efforts.<br /><br />This integrated method enhanced risk stratification and allowed earlier intervention through biopsy, counseling (including smoking cessation), and follow-up management. The study shows that combining AI risk assessment with LDCT imaging and liquid biopsy is feasible and scalable for community-level screening in India, addressing the critical need for improved early lung cancer detection.<br /><br />In conclusion, this multi-layered screening strategy optimizes precision, minimizes unnecessary invasive procedures, and holds significant promise to reduce lung cancer mortality by enabling tailored early interventions in high-risk populations. The pilot underscores the value of incorporating AI tools with established imaging and molecular diagnostics in resource-sensitive settings to improve public health outcomes.
